Your First Visit
We always want your visits to our office to be simple and streamlined, especially your first visit. To help make this a reality, our new patient paperwork is available for you to download, print, and fill out in the comfort and convenience of your home.
If you would prefer to fill out these forms in our office, please plan on arriving an additional 10-15 minutes early. If you have dental benefits that you intend to utilize, please bring your insurance card and a valid form of I.D. with you to your first appointment as well.
Dental Insurance
Our office accepts both in and out-of-network dental insurance, and we are happy to file, track, and update insurance claims on our patient’s behalf. Our knowledgeable team enjoys helping patients maximize their dental benefits to minimize what they have to spend out-of-pocket, and we are here to clarify any confusion that may arise and help you focus less on paperwork and more on your oral health.

Our In-House Membership Plan
Uninsured or underinsured? Our in-house plan is designed to help families fit high-quality care into their budgets, regardless of whether or not they have dental insurance. With our plan, there are no deductibles, annual maximums, waiting periods, pre-existing condition limitations, preauthorization requirements, or unexpected costs to worry about. Additional services like cosmetic and orthodontic care are also covered under our plans.

Terms and Limitations of the Plan
- This is a dental savings plan and is NOT dental insurance. It cannot be combined with any other dental insurance.
- This plan is non-transferable. Family members cannot be substituted for another family member.
- This plan is non-refundable, and no refunds will be given if the patient chooses not to use their dental plan.
- Rates are subject to change annually.
- Payments for services are due at time of service.

- Your effective date is the day you sign up and your renewal date is the same date every year.
